Hydrocortisone antagonise hypotensive effect of ACE inhibitors. Increased risk of hypokalaemia when hydrocortisone given with acetazolamide. Hydrocortisone antagonise hypotensive effect of alpha-blockers. Increased risk of hypokalaemia when hydrocortisone given with amphotericin so avoid concomitant use unless amphotericin needed to control reactions. Metabolism of hydrocortisone accelerated by carbamazepine (reduced effect). Hydrocortisone antagonise hypotensive effect of clonidine. Corticosteroids antagonise diuretic effect of diuretics. Increased risk of haematological toxicity when hydrocortisone given with methotrexate. Metabolism of hydrocortisone accelerated by phenytoin (reduced effect). Hydrocortisone may enhance or reduce anticoagulant effect of coumarins (high-dose corticosteroids enhance anticoagulant effect). Increased risk of gastro-intestinal bleeding and ulceration when hydrocortisone given with NSAIDs.
