Use of Drug

Paget's disease (a condition in which the bones are soft and weak and may be deformed, painful, or easily broken).

Hypercalcaemia of malignancy

Nitrogen containing bisphosphonate

Side Effects

Genaral- Generalized pain, fever, reactivation of viral diseases. Flu-like reaction may occur initially. 

Heart - High blood pressure, abnormal heart rate or rhythm, heart failure, syncope. 

CNS- Confusion, visual hallucinations, electrolyte imbalance, sleeplessness, nervousness, drowsiness, mental or mood changes. 

Gastrointestinal- Abdominal pain, loss of appetite, constipation, nausea, vomiting, diarrhea, mouth ulcer, gastrointestinal hemorrhage, osteonecrosis of jaw. 

Genitourinary- Increased blood urea, renal tubular disorders (RTD).

Skin- Rash, itching. 

Eye- Inflammation of eye tissues.

Blood- Decrease in red blood cells, white blood cells and platelets. 

Metabolic- Low thyroid hormone, decrease in calcium, potassium, magnesium, phosphate level in blood. 

Respiratory- Runny nose, upper respiratory infection, adult respiratory distress syndrome (ARDS and interstitial lung disease (ILD). 

Musculoskeletal Bone and muscle pain

Drug Dose Information

Intravenous
Paget's disease of bone
Adult: 30 mg daily for 3 consecutive days , may repeat course when clinically indicated. Alternatively, 30 mg wkly for 6 week  or 30 mg in the 1st week, then 60 mg every other week for 6 week may repeat course 6 mthly until remission of the disease is achieved, or relapse occurs. Max infusion rate: 60 mg/hr. Max total dose: 360 mg/course. 
Elderly: Initiate with lower doses.

Precautions

May increase the risk of heart failure in patients with cardiac disease.

Lactation

Risk of neurotoxicity

Patients with a history of these reactions associated with bisphosphonate therapy

How To Take

It available as a solution to inject into a vein, administered by a healthcare provider.

Storage

Injection: Store it at room temperature. After reconstitution: Stable for up to one day when refrigerated at 2-8°C.

Interactions

Additive hypocalcaemic effect with aminoglycosides Decreased absorption with antacids administer at a different time of day Decreased absorption with calcium salts Increases risk of GI adverse/toxic effects (especially GI ulcers) with NSAIDs May increase hypocalcaemic effect of phosphate supplements.
